4. Entering Data: HMIS Release of Information (ROI)

Release Of Information

Before entering any information into HMIS, projects must discuss and complete the HMIS Wyoming  Release of Information with each Head of Household that includes all household members. 

This document gives permission to share client information within HMIS with other participating agencies. 

If the client indicates "No" on the document, enter all household members information into HMIS, then contact the ICA Helpdesk immediately.

a. Add Release of Information                                                                                                                                                                                                                                                    

b. Click all clients in household

c. Provider: Defaults to what was selected for Enter Data As

d. Release Granted:  Yes or No

e. Start Date:  Defaults to current date or the date set for Back Date

f. End Date: One year from Start Date

g. Save Release Information
